Elder Care Coordinator

Sue Ann Daniel joined Nawrocki Center for Elder & Family Law in 2007 as an Elder Care Coordinator. She brought with her 17 years experience of social work experience. Sue Ann graduated with a Master of Social Work degree from Eastern Michigan University. She also has a Master of Science degree from Madonna in hospice Education. She has extensive experience working with elderly and families.

Sue Ann is responsible for a variety of duties, conducting assessments to identify problems and assist in solving them. Assisting families in identifying and arranging in-home help or other public/community services. Coordinating services with medical and health providers. Reviewing medical issues and offer referrals to other geriatric specialists to provide appropriate care while conserving financial resources. Providing support, guidance, and advocacy during a crisis. Helping with coordinating transfer and transportation of an older person to or from a retirement complex, assisted living facility, or nursing home. Providing education to client, families and community at large. Offering counseling and support to clients and families. Preparing and guiding clients and families through the Medicaid application process.

Sue Ann is active in Livingston Consortium on Aging, Aging Network and Alzheimer's Association - Great Lakes Chapter Regional Council.

At home, Sue Ann is wife of 18 years and a mother to two boys ages 16 and 13. She is an avid reader, gardener and scrapbooker.
